Who You Are

You are passionate about supporting children and young people with disabilities and their families to achieve the best possible outcomes. You possess strong skills in assessment, planning, and coordinating multi-agency support, ensuring that each child’s individual needs are fully understood and met. Committed to working collaboratively, inclusively, and creatively with children, families, and professionals from health, education, and social care sectors, you are resilient, reflective, and confident in managing complex situations and making child-centred decisions. You thrive in a supportive and specialist team environment and contribute to a culture of learning and continuous improvement. You are registered with Social Work England and hold a recognized Social Work qualification (Degree/DipSW/CQSW) with a minimum of 3 years post-qualifying experience.

What the Job Involves

This vital role involves working directly with children and young people who have a range of additional needs, as well as their families, to ensure they receive the right support at the right time. Your responsibilities will include completing holistic assessments, planning and coordinating services, and working collaboratively with families and partner agencies to promote positive outcomes and independence. Your focus will be on enabling children to thrive and reach their full potential, with their voice and experience at the centre of everything you do. You will work closely with families to build trusting relationships, ensuring they feel supported, valued, and part of the process. This is an exciting opportunity to make a genuine impact on the local authority and to help improve the lives of children and young people with disabilities across Herefordshire.
