We are seeking a highly motivated Social Worker to join our new Mental Health Social Work team in Adult Social Care. Representing social work in a network of professionals across an integrated setting, you will confidently manage your own social care workload

What You’ll Be Doing Are you passionate about providing high-quality, community-based mental health social care and support? Join our new Mental Health Social Work team in Adult Social care and collaborate with health, social and voluntary service colleagues to ensure residents with mental illness and their families receive timely, holistic assessments of needs, balancing risk and maximising protective factors. Your expertise will be crucial across different settings, representing your profession and supporting those in the community who need it most. In this role, you will manage your own workload and resources, respond to unexpected events and crises, and recognise and act on signs of harm, abuse, and neglect to safeguard vulnerable adults appropriately. You will support individuals with various mental health diagnoses using the Camden’s strength-based “What Matters” approach, focusing on core social work values, recovery, and trauma-informed care. This includes offering short-term support to address immediate concerns and developing long-term relationships to help individuals and their support networks maintain good mental health and build meaningful lives. This post is exempt from the Rehabilitation of Offenders Act and will require an Enhanced DBS.

All About You You must be a qualified Social Worker with suitable qualifying experience and be registered with Social Work England. A thorough understanding of the Care Act 2014 and current mental health legislation and its application is essential, as well as promoting the best interests of residents and carers. You should be able to assess situations, determine their nature and severity, and take appropriate action based on professional knowledge and experience. You will promote and embed a strengths-based approach to Social Work, contributing to a collaborative and reflective team environment. There will be opportunities to develop a wide range of skills, including coordinating Mental Health Act assessments and participating in Camden’s AMHP Duty Rota. If not already qualified, you should be willing to complete AMHP Training.

Summary of Duties and Responsibilities • Delivering support through Camden’s What Matters approach and connecting people within their communities. •  Participation within multidisciplinary teams and engagement with internal and external peers to support multi-disciplinary working to achieve best outcomes for people. • To act as the keyworker for people with an allocated worker drawing on care and support from a mental health team. •  Responsible for completing Care Act Assessments (including capacity assessments), care and support plans, and reviews in line with What Matters and in collaboration with other professionals to achieve excellent outcomes for people. •  Responsible for completing Court of Protection tasks. • Ensure people are safeguarded and protection plans are in place. • Carry out, review and record carers conversations (including carers support plans and carers annual reviews). • Creating and maintaining accurate case files and records in line with the local authority and mental health trust recording policies and procedures. • Take responsibility for and engage meaningfully in supervision and reflective practice. • Responsible for continued professional development and active engagement in the identification of own learning and development needs.

Work Environment: ASC Mental Health Hub is based at 5 Pancras Square.  The team shares the 7th floor with other ASC colleagues in Neighbourhoods and Learning Disabilities.  The location is convenient for travel throughout the catchment area, with good bus, train and tube links. Staff arrange to see service users at the office but also carry out home visits or arrange to see service users at various community locations dependent on their needs and preferences (for example GP surgeries, cafes or day centres). The role is primarily community-based which may be across several sites in Camden, including in hospitals and lone-working in people’s homes. There will be opportunities to work from home, up to a maximum of 2 days a week.

Technical Knowledge and Experience: Knowledge of IT systems: Carenotes , RIO and Mosaic are used in the team, but previous knowledge is not essential as training will be provided.
