This role involves working within the Later Life Team, focusing on providing direct support and interventions for adults aged over 67. You will assess and manage complex cases, collaborate with other health and care professionals, and develop personalized care plans to improve the quality of life for older adults. Your work will be crucial in easing transitions in later life, promoting independence, and safeguarding vulnerable adults.
