What the Job Involves
This role involves supporting young people leaving care in various aspects of their lives, including developing and reviewing pathway plans, managing caseloads, and attending key meetings. You will work closely with young people to ensure they can access suitable housing, financial systems, employment, education, and health care services. You will also be an advocate for young people, assisting them in navigating benefits agencies and accessing social and leisure opportunities. Collaboration with others in the care system and maintaining accurate records are essential responsibilities, as well as contributing to the service's improvement and participating in training and team activities.
