You are an experienced and motivated Mental Health Social Worker with a strong background in supporting adults experiencing mental ill health. You are confident, strengths-based in your practice, and able to manage a varied and fast-paced caseload. You possess a compassionate nature, upholding dignity, respect, and person-centered values.
What the Job Involves
The role involves working within a dynamic, multi-disciplinary environment, delivering high-quality social care support to adults in the community. Key responsibilities include handling referrals, conducting Care Act assessments and reviews, developing Section 117 aftercare plans, participating in MDT meetings, and planning safe discharges from hospital settings. You will also act as a Lead Enquiry Officer for safeguarding enquiries and conduct Carers Assessments, while contributing positively to team development.
Skills
- Social Work England registration
- Experience within a community mental health team
- Understanding of the Care Act and Mental Capacity Act
- Strong organizational and time management skills
- Experience with MDT working and use of IT systems
- Compassionate practice and respect for individuals
- Flexibility and enthusiasm for working with older adults
