You are a committed and skilled social worker with a passion for promoting the well-being of adults with care and support needs. You possess a strong understanding of Community Care Legislation and are registered or eligible for registration with Social Work England. You have a track record in completing assessments, support plans, and delivering social work services to diverse communities.
What the Job Involves
In this role, you will contribute to the operational delivery of an effective Adult Social Care Service. You will manage a complex caseload, work within legislative frameworks, and contribute to safeguarding, support planning, and promoting well-being. You will engage in multi-disciplinary forums, provide direct support to service users, and lead safeguarding enquiries. Collaboration with allied health professionals to assess and meet individual needs is a key component of the job.
