Who You Are

You are committed to supporting children, young people, and their families by providing high-quality, targeted outreach and home-based assistance. You have an understanding of a "whole family" approach and are passionate about preventing the need for statutory services while ensuring families can access universal services. Your interpersonal skills allow you to engage effectively with families, assess their needs, and coordinate plans to improve outcomes for children.

What the Job Involves

Your primary role is to take whole case responsibility for children and families, offering targeted support across age ranges (0-25 years, with specific responsibility for adults under SEND Code of Practice where applicable). You will manage cases stepping down from children's social care, facilitating Family Help assessments, and coordinating Team Around the Family meetings. Your responsibilities include providing intensive support, resolving urgent issues during home visits, and liaising with other professionals to formulate and review plans. You will also support the development of practical parenting strategies to enhance family relations and utilize outcome frameworks to gauge progress. Regular supervision, effective record-keeping, and proactive professional development are also integral parts of the role.
