You are a dedicated and compassionate professional committed to delivering exceptional adult social care services. Holding a Dip SW, BA, or MA in Social Work (or equivalent), you are either registered with Social Work England or due for registration shortly. With applied knowledge of Community Care Legislation, you excel in developing and managing support plans and have a track record of effective performance in diverse, multidisciplinary environments.
What the Job Involves
This role involves contributing to the effective delivery of adult social care services as part of an integrated team. You will be responsible for identifying individual needs, crafting, and maintaining support plans, handling complex caseloads, and ensuring compliance with legal standards and policies. You will work collaboratively with service users, families, and multidisciplinary teams to achieve positive outcomes, overseeing safeguarding, risk management, and acting as a care navigator. You will also take part in professional development, supervision, and contribute to the broader goals of the service and council.
