What the Job Involves

Your role involves working within a locality-based multi-disciplinary team, managing approximately four cases per week. You will conduct holistic person-centred assessments and plan and review support for individuals with various health conditions. Your focus will include assessing for complex and standard cases, recommending major adaptations, managing complex equipment, and working closely within the legal social care framework.
