You are committed to empowering service users and informal carers to achieve the highest level of independence and well-being possible.

What the Job Involves

As a Social Care Assistant, you will assess the needs of adults and older people with physical and mental disabilities, formulate and implement support plans, and organize appropriate solutions to meet their daily living needs. You will provide ongoing assistance, conduct risk and technology assessments, and work as part of a multi-disciplinary team to ensure the delivery of seamless support services. Furthermore, you will actively engage in managing cases, supporting service development, and assisting with welfare benefits and other financial matters.

Skills

  • Experience of working with individuals with social care needs
  • Understanding of the Care Act and eligibility criteria
  • Ability to work collaboratively in multi-agency environments
