What the job involves

Your primary responsibilities will include completing Care Act Assessments, Reviews, safeguarding, and care coordination under the CPA Framework. You will manage a caseload of 25 clients and participate in a Duty Rota, which may include screening, triaging, and handling new referrals. You will be an integral part of an integrated Mental Health and Wellness Team.
