What the job involves
This position involves providing support and care to adults with Learning Disabilities. You will develop and implement individual care plans, facilitate activities that promote independence and well-being, and work closely with a multidisciplinary team to ensure the best outcomes for individuals. The role requires active communication with family members and caregivers, as well as monitoring and reporting on progress and any concerns.
