You are a dedicated and compassionate social worker with a commitment to providing quality support within the framework of Community Mental Health. You have experience working in partnership with healthcare teams and have a deep understanding of the legislative requirements guiding mental health services.
What the job involves
Your role involves providing a comprehensive social work service as part of the Community Mental Health Team within the East London NHS Trust. You will deliver services in accordance with the Care Programme Approach and the Care Act, ensuring compliance with all relevant legislation. Your responsibilities include engaging with individual service users both in hospital settings and within the community, including placements, offering them advice, assistance, and support. Assessing, reviewing, and enhancing outcomes for service users and their carers will be a focus of your work.

- Understanding of the Care Programme Approach and Care Act
- Experience in integrated healthcare settings
- Strong interpersonal and communication skills
