Who You Are

You are an experienced agency social worker ready for a six-month placement within the Rhondda Cynon Taf West Learning Disability Team. You have a focus on supporting adults with learning disabilities, fostering an environment of safeguarding and wellbeing, and eager to join a well-established and supportive team.

What the job involves

Your primary responsibilities will include conducting reassessments and statutory reviews. You will deliver high-quality social work practices such as assessment, care planning, and review processes, adhering to the Social Services and Well-being (Wales) Act 2014. This role involves community-based work with a hybrid model allowing office days.

Key tasks include:

  • Safeguarding and promoting wellbeing through assessments and care planning
  • Balanced risk assessment and management promoting independence and choice
  • Identification and enhancement of individual strengths for positive outcomes
  • Collaboration with other professionals and agencies in a multidisciplinary approach
  • Completing and maintaining electronic records in line with departmental standards

Skills

  • Proficient in high-quality social work practice
  • Knowledge of Social Services and Well-being (Wales) Act 2014
  • Ability to assess and manage risks effectively
  • Strong interdisciplinary collaboration skills
  • Proficiency in maintaining detailed records
