What the job involves
As a Support Worker, you will provide 24-hour support in small, shared homes for adults with disabilities. Your responsibilities include assisting with daily activities such as personal care, communication, mobility, meal preparation, and community engagement. You will work from individual care plans to promote well-being and positive outcomes. Additionally, you will collaborate effectively with families, colleagues, and external professionals, ensuring accurate record-keeping and adherence to Social Care Wales standards.
