Who You Are
You are a compassionate and dedicated professional with a commitment to promoting well-being and protecting vulnerable adults. Holding a degree in Social Work or an equivalent qualification, you are registered with Social Work England and either have completed or are willing to undergo Best Interest Assessor training. Your strength lies in working with individuals and their carers to create and implement care plans that ensure their independence and safety.
What the Job Involves
This role involves assessing the needs of vulnerable adults and their carers, formulating and implementing person-centred care plans in line with the Care Act (2014), and safeguarding adults from abuse. You will work collaboratively with professionals, voluntary services, and private agencies to provide support, monitor services, and ensure they meet eligible needs. Responsibilities include managing resources efficiently, participating in multi-disciplinary teams, handling complaints, and keeping accurate client records. You'll be expected to adapt to new legislation and contribute to service development.
