What the Job Involves
As a social worker within the Assessment Teams, you will be responsible for conducting thorough assessments of individuals and families to determine their needs and eligibility for services. You will collaborate with a range of stakeholders, including healthcare professionals and community organizations, to develop and implement intervention plans that address identified needs. You will maintain accurate records and reports, and advocate for clients to ensure they receive appropriate support and resources.
