Who You Are
The ideal candidate is someone with in-depth knowledge of NHS Continuing Healthcare (CHC) eligibility and an understanding of the lawful limits of local authority provision under section 22 of the Care Act 2014. You should have extensive experience completing CHC assessments and the Decision Support Tool (DST) across a range of settings. You are a collaborative team player and an effective representative of the local authority perspective, capable of constructively challenging to support robust decision-making.
What the Job Involves
As a core member of the multi-disciplinary team, your role involves representing the local authority's perspective in the CHC assessment process, supporting robust decision-making, and evaluating potential joint funding arrangements. You will be responsible for completing CHC assessments and utilizing the Decision Support Tool (DST) to ensure appropriate healthcare provision.
