ASC - Adult Senior Social Worker

The role is in the front door offering focused short-term intervention to mitigate immediate risk and ensure that residents ‘wait well’ when further complex care management has been identified.  You will work alongside OTs in this new MDT approach, responding daily to referrals via the hub which deal with a broad range of Adult Social Care queries and referrals. You must have the ability and experience to maintain healthy relationships with partner agencies, particularly health, voluntary sector, and commissioned services. The ideal candidate should be conversant with different IT systems and know how to record information contemporaneously on social care systems. You will possess excellent communication, analytical and written skills and be a problem solver, as well as be effective at prioritising, meeting performance targets and managing systems. You will work to the requirements of the Care Act and Mental Capacity Act along with best practice.
