Role
- Working within strengths-based model of practice and holding a caseload to support residents to improve their independence and wellbeing by working closely with partners and maximising the use and benefit of community resources.
- Undertake strengths-based assessments, reviews and support planning that enable residents to maximise their independence, improve quality of life and build their resilience.
- Provide information and advice in addition to practical support to residents and carers, such as accessing assistive technology, aids and adaptations, benefit maximisation, Haringey connected communities and employment and volunteering opportunities.
- Assessment and coordination for short term care in addition to providing clients and carers with respite to reduce carer breakdown.
- Working collaboratively with clients, relatives, carers, and communities to help them make informed decisions, enabling them to clarify and express their needs and contribute to support planning.
- Liaise with the MDT, colleagues across the ICB, housing and social care partnership to gather information relevant to assessment and support planning activities.
- To keep accurate and timely electronic case records. To prepare documentation for panel application and other necessary documents as required.
- To work collaboratively with the Multi-Disciplinary Team to ensure clients voices are heard through assessment and in meeting forums. To work collaboratively to achieve best outcomes for those accessing services. 9. Participation in making safeguarding personal decisions and contribution to risk assessments and protection plans.
- To work with the National Framework of Continuing Health Care, to ensure individuals are informed in relation to the correct pathways for service provision.
- To undertake special projects, visits and group work in connection with their statutory duties.
- To undertake other temporary duties, consistent with the basic objectives and/or duties of the post.
- To always act in accordance with the council and departmental policy in particular the council’s equal opportunities policy.
- To undertake appropriate training as required.
