In this rewarding role, you will: Promote independence and wellbeing for adults and their carers Safeguard individuals at risk and respond to safeguarding concerns Assess needs in line with the Care Act (2014) Develop and deliver person-centred care and support plans Work collaboratively with service users, carers, and partner agencies Monitor and review care to ensure high-quality, effective support Contribute to service development and continuous improvement
