Community Social Work Social Worker

Working as a social worker care coordinator/Key worker. Carrying caseload of clients and supporting them with mental state follow up and monitoring and ensuring KPIs are up to date, carers are supported as well as proper link up with other agencies within and outside NHS and social care. Completing Care Act Assessment, carer assessment, social circumstances report, MHAA/MCA and other social work related engagement and support withing the team. Others as required by the team.
