ASW Mental Health Social Worker WCR 3

General

Working closely with health colleagues in the Joint Homelessness Team based in Vauxhall Bridge Road. Supervised by the senior social worker, the social worker will carry out local authority duties in relation to older adults with a combination of mental and physical health problems. The duties include: Carrying out Care Act Assessments and reviews, Setting up care packages and reviewing the care packages, Hospital Discharge planning and where necessary taking cases to funding panels for appropriate intensive supportive care packages or placements. Carrying out assessments under the mental capacity Act and fulfilling safeguarding duties. Multidisciplinary working and joint visits with mental health colleagues.

Assessment Skills Good listening skills Ability to build rapport with older adults who may have a functional mental illness or a cognitive impairment. Multidisciplinary working skills Time management and Good IT skills.
