Mental Health Social Worker / Recovery House Link Social Worker Westminster City Council Rate: £32 per hour (Umbrella) Hours: 36 hours per week, Monday to Friday, 9:00am – 5:00pm Contract: Agency / Interim Locations: Office base: 190 Vauxhall Bridge Road, London SW1V Service base: Recovery House, Pimlico Hybrid working is available, subject to agreement with the Senior Social Worker and Team Manager. The Role Westminster City Council Adult Social Care is seeking an experienced Mental Health Social Worker to join the South Locality Adult Social Care Mental Health Team, with a dedicated link role to an imminently opening 5-bed Recovery House in Pimlico. The postholder will report to a Senior Social Worker and will work flexibly between the Vauxhall office base, the Recovery House in Pimlico, and the community. The role is central to supporting recovery-focused mental health pathways and effective partnership working with health colleagues. Key Responsibilities Act as the named/link social worker for the Pimlico Recovery House Provide social work expertise to Recovery House staff, including OTs, support workers and the Mental Health Outreach Worker Act as the key liaison with Trust PCN teams for Recovery House residents Participate in the South Locality daily duty rota, responding to incoming referrals and enquiries Undertake Care Act strengths-based needs assessments Complete Care Act care and support plans, reviews and reassessments Apply for and coordinate funding for placements Develop and review Section 117 aftercare plans in collaboration with health colleagues Attend MDT ward rounds and community MDT meetings Lead discharge planning from acute and psychiatric hospital settings Act as Lead Enquiry Officer (LEO) for safeguarding enquiries Undertake carers’ assessments under the Care Act Complete Mental Capacity Act assessments and best interests decision-making Attend team meetings and contribute to service delivery and development

The Team The role sits within the South Locality Adult Social Care Mental Health Team, Westminster City Council, working closely with health, housing and community partners.
