Role
To perform all the functions of the post to comply with legislation and within the policies, aims, guidelines and resource constraints of the Social Care & Housing Service.
Prepare for, and work with individuals, families and others, listening to understand their needs and circumstances, what matters to them and when necessary jointly agreeing a course of action.
Develop support plans with individuals and others built upon self-reliance in a positive and empowering way.
Demonstrate effective interagency, multidisciplinary and partnership working to provide a best practice service.
Plan, carry out, review and evaluate social work practice with individuals, families and others.
Ensure the implementation of agreed care and support plans in liaison with other agencies and service providers, regularly monitoring, reviewing and revising as appropriate.
Address behaviour in the individual and others in a constructive way in order to minimise risk of harm and prevent abuse.
Advocate for and with individuals, families and relevant others.
Work with individuals and others to select the best form of representation according to the circumstances.
Assess, manage and monitor risks to individuals, families, self and others, whilst not discouraging positive risk taking.
Take immediate and proportionate action to deal with/ address any behaviour that presents a risk.
Manage and be accountable, with supervision and support, for your own social work practice and others if commensurate with grade.
Manage an individual case load and associated administrative tasks in an efficient and effective manner, using IT systems and software as appropriate.
Provide clear, proportionate and accurate reports to the standards within Torfaen County Borough Council and within agreed timescales.
Adhere to the department’s policies on data protection, record-keeping and confidentiality .
Proactively participate in individual supervision and performance appraisal reviews.
Contribute positively to the team through working in co-operation with colleagues and the team leads.
As directed and commensurate with grade Contribute to the professional development of less experienced colleagues acting as a supervisor, mentor and/ or coach for other’s practice (such as students, volunteers and support staff).
Comply with and support others to observe Health and Safety procedures and processes.
Demonstrate professional competence in Social Work practice.
Demonstrate a commitment to continuous professional development, according to the requirements of the Care Council for Wales.
Participate in and inform service and practice reviews.
Support and appropriately influence the development of best practice systems, processes and procedures.
Undertake other such related duties as are required from time to time by the Chief Officer or their representative.
