You are an experienced Occupational Therapist with a strong understanding of the Care Act 2014 and the Discharge to Assess model. You are committed to promoting independence and enhancing the wellbeing of individuals undergoing hospital discharge. You have excellent assessment skills and a collaborative approach to multidisciplinary team working.
What the job involves
As an Occupational Therapist in the Hospital Discharge Team, you will perform specialist assessments and interventions to facilitate safe and effective hospital discharges. You'll be responsible for assessing residents' functional abilities and recommending equipment or adaptations to support independence. You will contribute to the Discharge to Assess pathway, provide input in discharge planning meetings, and deliver training to colleagues on occupational therapy practices. The role involves providing hospital duty cover and responding to urgent referrals to support system flow and prevent readmissions.
