Hybrid working role. Shift working on a rota - weekend cover required every 7 weeks.

Reablement OT Job Description:

The role of Reablement OT is vital for helping individuals regain independence and confidence in their daily lives. This role is ideal for someone passionate about recovery-focused care and committed to enabling people to live safely and independently in their own homes. As a Reablement OT, you will use your professional expertise to assess, implement, and deliver personalised reablement programmes that support service users and their carers in achieving meaningful outcomes. You will work within the framework of relevant legislation and departmental responsibilities, ensuring that interventions are strength-based, person-centred, and tailored to individual needs. You will be part of a multi-disciplinary team (MDT), collaborating with health and social care professionals to support individuals with a wide range of physical, cognitive, and social challenges. Your work will be central to: Driving recovery and functional improvement Reducing reliance on long-term care Recommending proportionate care where long term needs are identified Facilitating timely and safe hospital discharges Promoting independence and wellbeing in the community

In addition to your clinical responsibilities, you will support the development and delivery of the service, assisting the manager with delegated tasks that contribute to service improvement and innovation.

Key responsibilities include: To work as a member of a community based multidisciplinary team.  Use Occupational therapy expertise to assess, implement, evaluate and review plans for service users and carers, enabling them to achieve maximum independence in their own homes. Provide a broad range of support to clients who have physical or mental health problems, learning difficulties, dependencies or who are elderly. Help to develop and/or maintain resident’s independence using a strengths based approach. Requirement to work on a shift basis including bank holidays, evenings and weekends.

Main Accountabilities: The postholder will identify the needs of the service users and carers within the context of their own homes and within available resource.

To involve carers in this process and take into account their needs as well as those of the service users’ to ensure best outcomes. To manage autonomously a clinical caseload of clients at a specialist level, assessing and treating clients and maintaining clinical records. Participate in clinical supervision and appraisals, and supervise OT Assistants according to level of expertise. To ensure that all activities are informed by up to date manual handling advice and training. To set SMART goals to ensure therapeutic interventions are effective and to improve client flow through the service. Promote the use of Assistive Technology and AskSara to enhance independence and reduce reliance on formal care services. To work collaboratively with health and social care community and acute colleagues, including Local Access Point and Discharge to Assess teams. To work with other members of the MDT to ensure effective assessment, treatment, management of care pathways and transfers of care / discharges. To support the development of specialist programmes of assessment, treatment and intervention for clients within a community setting. To be responsible for the issue and safe use of equipment used in your area of work and to adhere to the appropriate service policies. To participate in the team CPD activities, supervision process and peer review, maintaining an up to date CPD portfolio  This should demonstrate responsibility or maintaining own competency to practice. To carry out other tasks as are commensurate with the post according to OT core skills.
