Case Manager (SO1)

Islington Council is seeking motivated and compassionate Adult Social Care Case Managers to join its Adult Social Care service. This is an exciting opportunity to support adults, carers and families by coordinating care, promoting independence and delivering person-centred, strengths-based interventions. As a Case Manager, you will work within a multi-disciplinary team alongside social workers, occupational therapists and other professionals. You will manage a varied caseload, undertake assessments, support planning and reviews, coordinate services, and connect residents with community-based resources to help them live independently and achieve positive outcomes.

Key Responsibilities Manage a caseload of adults with a range of care and support needs. Conduct assessments, reviews and support planning using a strengths-based approach. Provide information, advice, signposting and early intervention support. Work collaboratively with residents, carers, community organisations and partner agencies. Identify and respond appropriately to safeguarding concerns. Maintain accurate case records and documentation. Support service users to access community resources, assistive technology and preventative services. Participate in duty rotas and respond to urgent situations when required. What We're Looking For

Essential Requirements: Level 3 Qualification in Health and Social Care (or willingness to work towards achieving it). Experience working with adults within social care, health, or the voluntary/community sector. Knowledge of adult social care legislation, safeguarding and strengths-based practice. Strong communication, assessment and case recording skills. Experience working effectively with multidisciplinary teams and external partners.
