Support Worker – Day Opportunities Job Summary The Support Worker provides person-centred care and support to adults with learning disabilities, sensory disabilities, autism, mental health needs. The role focuses on promoting independence, strengths-based practice, community inclusion, wellbeing, and helping residents achieve their individual goals Purpose of the Job To deliver care and support in accordance with individual support plans, maximising independence, promoting wellbeing, managing risks appropriately, and enabling people to live healthy, fulfilling lives while remaining connected to their communities.
Main Areas of Responsibility Deliver person-centred, strengths-based care and support. Support people with personal care, domestic tasks and independent living skills. Promote independence and choice in all aspects of support. Lead and support activities, groups and community engagement opportunities. Support individuals with medication management in line with support plans and policies. Support individuals with nutritional and dietary requirements. Attend and contribute to multidisciplinary meetings. Maintain accurate records and documentation. Work in partnership with carers, families, community organisations and professionals.
Main Accountabilities Deliver support plans safely and effectively. Report safeguarding concerns immediately. Implement the Care Act 2014 in practice. Support positive risk-taking while promoting independence. Provide updates on individuals' goals and progress. Liaise with health and social care professionals. Record information accurately using electronic systems. Maintain infection control, health and safety, GDPR and safeguarding standards. Experience NVQ Level 2 or QCF Level 3 (or equivalent) in Health & Social Care. Experience supporting adults with complex needs, learning disabilities, sensory or physical disabilities, mental health needs, autism, Experience working with diverse communities. Experience applying strengths-based approaches in practice. Knowledge of safeguarding and Making Safeguarding Personal Knowledge of the Care Act 2014 and Mental Capacity Act. Experience working with multidisciplinary teams including social workers, health professionals, housing and GPs. Ability to record information accurately and maintain support records. Nice-to-Have Skills & Experience Autism support experience. Learning disability and profound multiple learning disability (PMLD) experience. Makaton, Intensive Interaction or alternative communication methods. Experience leading activities and group sessions. Travel training and community access support. Medication administration experience. Experience using electronic care recording systems and IT. Ability to support people with behaviours that may challenge.
