Role
The Hospital Social Work Team effects safe hospital discharges for patients - particularly using the Discharge to Assess Model. We are looking for adaptable social workers to work in our A&E, short stay wards as well as residents on admission at particularly the Royal London Hospital. The work is fast paced with a quick turnover of assessments and caseload. Our cases are transferred to the appropriate locality teams once the service user is discharged safely therefore, we do not hold on to cases and we do not operate a waiting list system. Hospital D2A experience is a must. The team is very supportive with a strong management system.
