Outreach Support Worker (Scale 5)

A part-time Support Worker is required within Outreach to ensure the service can continue to meet the needs of individuals living at Vincent House and provide safe, consistent support. The requirements of the post are to support adults with a learning disability who live in their own homes, helping them to maximise their independence, work towards the agreed outcomes and goals set out in their reviews and support plans, mitigate identified risks, and carry out key worker responsibilities.
working up to 35hrs weekly on a 4 week rolling rota working 5 days per week to include alternate weekends
