What the job involves

This role primarily involves the assessment of risk and care needs for service users and carers, ensuring appropriate actions are taken, monitored, and reviewed. You will be responsible for managing a caseload, working within community care and integrated care programs, conducting statutory reviews, and protecting individuals from harm. The job requires engagement with individuals refusing services, crisis management, preparation of reports, and needs-led service facilitation. Additional duties include supervising unqualified staff or students, acting as a link worker with other agencies, developing projects, and protecting service users' property when necessary.
