Role

Who You Are

You are a dedicated professional with a Dip SW, BA, or MA in Social Work or equivalent, either registered with Social Work England or in the final stages of obtaining this qualification and registration. You have a strong applied knowledge of community care legislation, including the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983. Your experience includes completing assessments and support plans using information technology.

What the Job Involves

This role contributes to the operational delivery of an effective Adult Social Care Service within an integrated team. Responsibilities include identifying individual needs through assessment, planning and co-designing individual support plans, managing complex and varied caseloads, ensuring regular monitoring and reviews of support plans, and making appropriate assessments and acting as a care navigator. You will be involved in safeguarding, support planning, promoting adult wellbeing, and working cooperatively across multi-agency boundaries. Additional tasks include direct work with service users, developing inclusive practice, and contributing to the service's overall development.
