Hospital Social Worker (Level 7)

Who You Are You are an experienced social worker with a strong background in health or social care settings. You have experience in risk assessments, Mental Capacity and Best Interests assessments, and a comprehensive understanding of the Care Act. You are adept at managing complex workloads and are committed to equality and diversity.

What the Job Involves The role involves providing holistic assessment, care and support planning, and review activities for residents and their carers. You will work within a community team, applying a strengths-based, person-centered model of social care. You will handle a caseload of residents, conduct assessments, and design care plans according to legislation and best practices. You will collaborate with various professionals to meet the needs of service users and contribute to joint working practices. Additionally, you will ensure data is recorded accurately, engage in evidence-based practice, and maintain a safe working environment.
