Wrexham County Borough Council is seeking an experienced Agency Social Worker to join the Team for Older People. The successful candidate should be a qualified Social Worker with registration with Social Care Wales, not just Social Work England. Applicants should have experience in adult social work assessments, care planning, and reviews, as well as working with older people and undertaking Mental Capacity Assessments and Best Interest decisions. Additionally, candidates should have experience in safeguarding adults. The ability to travel throughout the county and living within 45 minutes of Wrexham is required.
What the Job Involves
This hybrid role combines office-based work, community and hospital visits, and home-based work. The candidate will manage a caseload of older adults, working with citizens, their families, carers, and partner agencies to promote independence and wellbeing. Responsibilities include participating in the team's duty rota for new referrals, conducting strengths-based assessments, developing care and support plans, and responding to safeguarding concerns. Effective risk assessment, decision-making skills, and high-quality reporting are required.
