What the job involves
As a Social Worker in the Transitions and Leaving Care Service, you will provide high-quality support for young people transitioning to independence. Your responsibilities include conducting assessments, developing care plans, and advocating for young people's needs. You will support 16- and 17-year-olds in our care, ensuring they develop the skills and resilience needed for independent living. The role involves collaborating with external agencies, conducting regular visits, and maintaining accurate case records.
