What the Job Involves
As a Social Worker in the Transitions and Leaving Care Service, you will provide high quality and effective services to support young people transitioning to independence. Responsibilities include conducting assessments, developing care plans, providing guidance and support, advocating for young people's needs, and ensuring comprehensive and person-centred pathway plans are developed. You will promote the participation and voice of young people, liaise with external agencies for a holistic support network, and maintain accurate case records.

- Social Work qualification
- Social Work England registration
- Experience in safeguarding
- Experience with young people in care
- Knowledge of relevant legislation
- Effective communication and advocacy skills
