You have experience in social care or healthcare environments, demonstrating effective communication, risk management, and analytical skills. You are committed to ongoing professional development and have a full EU driving license.
What the job involves
This role involves working within the Adult Social Care team to assess and review the complex needs of customers and their carers according to the Care Act 2014. You will provide advice, offer guidance, and facilitate access to support services. The position requires undertaking risk assessments, developing safeguarding plans, maintaining accurate records, and collaborating with varied stakeholders. You will work to maximize independence and wellbeing through assistive technology and person-centered approaches, supporting positive risk enablement, and contributing to the professional development of less experienced colleagues.
