You should have the ability to conduct strength-based assessments and develop support plans that identify individual, community, and universal strengths.
What the Job Involves
The role involves completing timely statutory reviews with adult residents, assisting individuals, carers, and families by implementing a strength-based, person-centered approach. You will create creative and personalized solutions to help people manage their lives independently, ensuring that agreed outcomes are met. Duties include encouraging direct payments, performing ongoing assessments of service users' needs, assessing risks, identifying and addressing safeguarding issues, and referring to appropriate staff or agencies to protect clients' welfare and optimize support quality.
