A motivated and experienced Social Worker who is committed to promoting independence, safeguarding vulnerable adults, and delivering person-centred care. The successful candidate will be confident working in a multi-disciplinary environment and supporting service improvement and professional development.
What the Job Involves
This role is responsible for supporting adults with complex needs by carrying out assessments, developing support plans, and coordinating services that promote independence, wellbeing, and positive outcomes. The post holder works within a strength-based, person-centred approach, helping individuals make informed choices while managing risks and safeguarding concerns. Responsibilities include managing a complex caseload, completing health and social care assessments, identifying reablement opportunities, undertaking safeguarding enquiries, supporting discharge planning, providing advice, and contributing to mentoring and practice development.
Skills
- Qualified Social Worker registered with Social Work England
- At least two years' post-qualification experience
- Strong knowledge of social care legislation and services
- Independent working and priority management
- Excellent communication and problem-solving skills
- Ideally hospital social work experience
