Who You Are
You are an experienced and qualified Social Worker with a registration with Social Work England and the legal right to work in the UK. You possess excellent communication skills, both written and oral, and have good assessment and case management skills. You have a strong knowledge of Personal Budgets/Self Directed Support and sound knowledge of relevant legislation, social care initiatives, and priorities.
What the job involves
You will be part of a friendly team of Social Workers and Support Coordinators, providing interventions for adults with long-term care needs, primarily focusing on older adults and individuals with physical disabilities. Your responsibilities include facilitating effective assessment and support planning, undertaking reviews and safeguarding investigations, and care managing a varied caseload. You will work collaboratively with health and social care colleagues to ensure person-centered multidisciplinary support planning and make effective use of resources through strength-based care planning.
