A qualified Social Worker with a strong commitment to promoting independence and reducing long-term dependency among adults with learning disabilities, autism, and complex needs. You value person-centred, strengths-based practice and have a deep understanding of relevant legislation and frameworks.
What the job involves
This role involves undertaking high-quality assessments, reviews, and care planning in compliance with the Care Act 2014. You will develop and implement person-centred support plans, work collaboratively with multidisciplinary teams, and contribute to service improvement initiatives. A key focus is on progression planning to promote skill development and increased independence for service users.
Skills
- Qualified Social Worker with Social Work England registration
- Knowledge of Care Act 2014 and Mental Capacity Act 2005
- Experience with learning disabilities and autism
- Analytical assessment skills
- Strong communication skills
- Autonomous work ability
- Provider negotiation skills
- Experience in transitions and progression-focused models
- Understanding of strengths-based practice frameworks
