What the job involves
The role involves performing comprehensive Care Act reviews and reassessments for adults with learning disabilities, conducting face-to-face visits across various settings, gathering and analyzing evidence to assess care and support needs, and producing high-quality documentation. The position requires collaboration with multidisciplinary teams to facilitate evidence-based decision-making and participating in funding reviews and CHC processes. The social worker must maintain up-to-date knowledge of legislation, policy, and best practices, working autonomously while managing a demanding workload.
