Who You Are
We are seeking experienced Locum Social Care Practitioners to work with adults receiving support from the London Borough of Bromley. The ideal candidate should have experience in care management settings and a strong understanding of adult social care. You should be confident working autonomously and managing a complex caseload.
What the Job Involves
As part of the Coordination and Review Team, you will carry out assessments and reviews under the Care Act 2014, ensuring care and support plans align with agreed outcomes. You will work collaboratively with care providers, identify and manage risks, undertake Mental Capacity Assessments, and maintain comprehensive case records. The role may involve traveling within and outside the borough for reviews and involves partnership work with housing providers and health professionals.
