Who You Are
As a Locum Social Care Practitioner, you are an experienced professional in care management settings, particularly in adult social care. You are confident in working autonomously and managing a complex caseload, with a strong understanding of the Care Act 2014, Mental Capacity Act 2005, and safeguarding duties.
What the job involves
In this role, you will undertake planned reviews for adults receiving support from the London Borough of Bromley. Responsibilities include completing Care Act needs assessments and annual reviews, supporting residents by ensuring care plans reflect agreed outcomes, collaborating with care providers, undertaking Mental Capacity Assessments, and maintaining accurate case records. You will also work creatively to provide cost-effective services, support qualified staff with practical tasks, and work in partnership with various stakeholders to ensure continuity of care.
