Who You Are
We are seeking experienced Locum Social Care Practitioners to join the Coordination and Review Team at the London Borough of Bromley. You should have a deep understanding of care management settings, recent experience in adult social care, and a strong working knowledge of key legislation like the Care Act 2014 and Mental Capacity Act 2005. You are confident working autonomously, managing complex caseloads, and experienced in supporting service transitions and partnership working.
What the job involves
The role involves carrying out assessments and reviews in accordance with the Care Act 2014 for adults receiving support in various settings, including care homes and domiciliary care services. You will complete Care Act needs assessments, support continuity of care, undertake Mental Capacity Assessments, and ensure safeguarding measures are in place. You will work with care providers and health professionals, offering practical support and ensuring the effective and timely delivery of services to adults and carers within the borough and potentially beyond.
