Role

Who You Are

We are looking for conscientious and resilient Social Work England registered practitioners with a minimum of two years' post-qualification experience. Candidates should have experience within mental health services, hospital discharge, or integrated health and social care settings. You will have excellent assessment, risk management, and multi-agency working skills, together with the ability to build effective relationships with individuals, families, professionals, and partner organisations.

What the job involves

You will manage a caseload of adults requiring assessment, care and support planning, and the coordination of services to facilitate safe discharge from inpatient mental health settings. Working within a multidisciplinary environment, you will undertake Care Act assessments, develop person-centred support plans, and coordinate services that promote independence, reduce delays, and achieve positive outcomes for individuals. The role requires close partnership working with NHS colleagues, families, community services, and care providers to ensure effective discharge arrangements are in place. You will be responsible for identifying and managing risk, responding appropriately to safeguarding concerns, arranging and reviewing care and support packages, and maintaining accurate records to support high-quality statutory social work practice.
