Who You Are
We are looking for a qualified social worker with mental health experience to join our community team at Carlton Parade. You should have the ability to work within a multidisciplinary team and be adaptable to cover for a team member on unpaid leave.
What the job involves
The role includes managing a caseload of clients, supporting and coordinating care, and occasionally acting "on duty" as part of a team rota. Responsibilities include all duties under the Care Act, such as Care Act assessments, carer assessments, and adult safeguarding. The team, "ADAPT East," supports residents with affective disorders, eating disorders, hoarding, anxiety, or a history of trauma in their homes or placements, facilitating hospital discharge as necessary. The position requires working from the base at least 4 days a week.
