You are passionate about supporting children and young people with disabilities and their families to achieve the best possible outcomes. You are skilled in assessment, planning, and coordinating multi-agency support, ensuring that each child’s individual needs are fully understood and met. You are committed to working collaboratively, inclusively, and creatively with children, families, and professionals from health, education, and social care. You are resilient, reflective, and confident in managing complex situations and making child-centred decisions. You thrive in a supportive and specialist team environment, contributing to a culture of learning and continuous improvement. You are registered with Social Work England and hold a recognized Social Work qualification with a minimum of 3 years post-qualifying experience.

What the Job Involves

In this role, you will work directly with children and young people who have a range of additional needs, along with their families, to ensure they receive the right support at the right time. You will complete holistic assessments, plan and coordinate services, and work collaboratively with families and partner agencies to promote positive outcomes and independence. Your work will focus on enabling children to thrive and reach their full potential, with their voice and experience at the center of everything you do. You will be part of the Children with Disabilities Team, contributing to a restorative approach to social work within Herefordshire.
