Role

Who You Are

We’re looking for social workers who are experienced in managing complex Older People and Physical Disabilities (OPPD) caseloads with a minimum of 2 years post-ASYE experience. You should be skilled in safeguarding investigations, strength-based assessments, and confident in using IT databases like Mosaic or Liquid Logic. You must possess excellent communication skills to build trust and work collaboratively with individuals, carers, and professionals. A creative problem solver who can think innovatively to meet service users’ needs, and committed to upholding Southwark’s values of inclusion, empowerment, and accountability.

What the job involves

As a Social Worker within the Assessment & Intake Team, you will hold a varied caseload, completing Care Act Assessments and Carers Assessments. You will undertake unscheduled reviews of care, present packages for funding approval, and complete Mental Capacity Assessments and Best Interest Decisions. Duties include carrying out initial safeguarding enquiries and short-term intervention work (up to eight - twelve weeks), arranging care home placements, and working closely with families and community resources to promote independence. You will collaborate with Occupational Therapists, health partners, and community agencies to deliver effective, person-centered support and play a key role in stabilizing complex cases for transfer to longer-term teams where appropriate.
